Certified copies can be purchased from the health department approximately 6-8 weeks following the birth or death.
Currently, most certified certificates are available for deaths occurring after 1983 and births occurring after 1920.
Fees are as follows:
- Death Certificates: $14.00 for the first copy and $11.00 for each subsequent copy purchased at the same time as the first copy.
- Birth Certificates: $15.00 for each certified copy.
Valid identification required. Local Checks only.
Please be aware that we cannot make any changes or corrections on vital records. Corrections must be made by the Bureau of Vital Records in Jefferson City, MO.

Ste. Genevieve Health Dept. has two passport agents who can accept new applications and/or review renewal applications. All forms can be picked up through the Health Department or printed out through travel.state.gov. We do also offer passport photo service for $15.
Appointments are required! Please call to make an appointment and confirm an acceptance agent is available.
When applying for a NEW passport, the following documents are required:
- DS-11 Form for New passport (application form filled out in black ink only)
- Photo ID (for anyone 16 and older)
- For anyone under 16, copies of both parents id (Both Parents must be present at the same visit)
- Certified copy of Birth Certificate
- Passport Photo
- 2 forms of payment.
- Check or Money Order made out to “US DEPT. OF STATE”
- Check/ Money Order/ Cash for “Health Dept” for execution fee

Cholesterol/ Lipid Panel
Other Lab Tests
Blood can be drawn for some other lab tests. The specimens are mailed to Quest and the physician receives the lab results in about one week. Patients must have a signed order from their physician and an appointment is required. Patients are required to pay for these tests prior to having blood drawn.
Child Care Health Consultant:
The safety and well being of their children while in daycare is very important to parents. The health department provides health and safety education to day care providers to improve the health status and ensure safety of children in child care facilities. The promotion of health and safety occurs through on-site assessments of the facility, child care staff and disease prevention practices. Health promotion/educational presentations are offered to staff, children and parents. Staff education is rewarded in clock hours for daycare licensure. Call the Health Dept. at (573)883-7411 to register for the classes when they are available.
CPR/ First Aid:
Our nursing staff offers different CPR classes throughout the year through the American Heart Association. Once classes are available they will be posted along with the cost for each class. For questions call the office.
